ONE-D (One Day) Accelerated TMS

What Is ONE-D Accelerated TMS?

ONE-D Transcranial Magnetic Stimulation

One-Day TMS condenses the FDA-cleared intermittent theta-burst stimulation (iTBS) protocol into a carefully spaced series of 20 sessions delivered within a single day. While this accelerated schedule is not FDA-approved, the iTBS protocol itself is FDA-cleared for depression, and early research supports the feasibility, safety, and potential efficacy of condensed treatment formats.

The ONE-D model draws from emerging clinical data, including a 2025 open-label case series of 32 adults with depression treated with a single-day 20-session protocol combined with neuroplasticity-enhancing medication. This study found the regimen to be “feasible, safe, well tolerated, and associated with clinically meaningful improvement”* with effects sustained at 3–6 months.

This format may be ideal for individuals who want a highly time-efficient treatment experience or who face complex scheduling demands—and who want to pursue a non-medication, neuroscience-guided pathway with close clinical supervision.

Accelerated TMS is supported by a growing body of neuroscience research showing that multiple iTBS sessions delivered within a condensed timeframe may deliver attractive convenience with similar efficacy.

Note that although the evidence is promising, accelerated TMS schedules—including single-day protocols—are not FDA-approved and remain investigational.

Who Might Consider One-day TMS?

One-Day TMS may be an appropriate fit if you:

  • Want a focused, time-efficient therapeutic approach

  • Prefer to minimize time away from work, caregiving, or travel

  • Are exploring non-medication or augmentation-based treatment pathways

  • Need a structured, neuroscience-driven intervention delivered in a single day

  • Have not had success with prior medications or talk therapy

What to Expect: ONE-D Protocol

The One-Day format includes:

  • 20 iTBS sessions delivered over the course of one day

  • ~30-minute spacing between sessions (mirroring intervals used in accelerated research)

  • 10-minute treatment window per session (3-minute iTBS + prep)

  • Left DLPFC targeting

  • Continuous clinical monitoring by trained TMS clinicians

A full treatment day typically lasts 10–12 hours. The schedule begins at 8:00 AM with check-in, vitals, and baseline assessments. The first iTBS session begins around 9:00 AM, with sessions repeated every 30 minutes through the day. A mid-day meal break is included. Treatment finishes in the early evening with a post-treatment evaluation and next-day follow-up plan.

Dr. Lewis is both super smart and on the cutting edge of research in the field, and a thoughtful compassionate listener. She is great at getting to the nut of any issue and makes thoughtful recommendations for how to move forward. I've sent her multiple patients who give her great reviews and the kids she's worked with think she is super-cool.
