What Is Maintenance TMS?

Maintenance Transcranial Magnetic Stimulation

Maintenance TMS is a structured, monthly approach designed to help you protect the improvements you achieved during your initial TMS treatment.

Depression is often episodic, and without ongoing support, symptoms can return. Maintenance TMS provides a predictable, non-medication strategy to keep you stable, supported, and feeling like yourself.

At Integrative Mind Psychiatry & TMS, we offer Clustered Monthly Maintenance TMS, a protocol backed by clinical studies and designed to fit real-life schedules.

Why People Choose Maintenance TMS

Simple Science. Powerful Results.

Maintenance TMS is ideal for people who:

  • Responded well to an acute TMS course
  • Have a history of recurring depressive episodes
  • Want a non-pharmacologic relapse-prevention plan
  • Prefer to minimize medication adjustments
  • Need structured, ongoing care to stay well

How Maintenance TMS Works

Maintenance TMS is delivered in monthly clusters — brief, efficient treatment blocks that support brain stability without weekly visits.

Each monthly maintenance cycle includes:

  • 5–10 sessions per month
  • Completed over 3–5 consecutive days
  • Repeats once per month

This model provides a strong “booster” effect that reinforces the neuroplastic gains from your initial treatment.

What to Expect Each Month

During each monthly cluster:

  • You’ll receive the same or similar TMS protocol used during your acute course
  • Sessions last 10–20 minutes depending on your mapping
  • You’ll complete a brief clinical check-in
  • You can return to normal activities immediately after
  • Most patients incorporate maintenance easily into their work and family routines

The Evidence Behind Maintenance TMS

Clinical studies support the use of ongoing stimulation to prevent depressive relapse.

A randomized controlled trial by Wang et al., 2017 showed relapse rates of:

  • 15.9% — rTMS + antidepressants
  • 24.2% — rTMS alone
  • 44.4% — antidepressants alone

Other studies support the effectiveness of clustered monthly maintenance, demonstrating sustained symptom relief and improved long-term outcomes.

Is Maintenance TMS Right for You?

Maintenance TMS may be a good fit if:

  • You responded to TMS but want to prevent recurrence
  • You’ve had depressive relapses in the past
  • You feel subtle symptoms returning
  • You prefer to avoid medication changes
  • You want a proactive, long-term plan

If you’re unsure, our clinicians will help recommend whether maintenance makes sense based on your history, symptom pattern, and goals.
